The twinning will focus on transferring the ReMemberMe system to address cognitive decline. This solution enables geriatric patients to self-administer reports and
share their autoscore with healthcare professionals for remote monitoring and personalised recommendations. This application allows input from caregivers, relatives, and healthcare professionals according to the users interests and needs. Caregivers in Greece will receive training on communicating with their patients via the ReMemberMe system.
