Short description
ICT for empowerment and self-management for patients with type 2 diabetes. The tool´s objectives include patient´s safety and protection, shared care plan between patients and providers, interoperability, predictive and prognostic indicators. The practice is based on delivering high quality services to patients and carers via Shared Care Plans, education and training, reports and data input
