Short description

HappyAir is a hybrid digital health platform (web + mobile app) integrating:

  • Patient self-management tools. Digital tools self-care and symptom monitoring. (e.g. Check@ir).
  • Remote clinical monitoring.
  • Social community support.
  • Tele-coaching by respiratory specialists.
  • Data analytics dashboard for HCPs.

Description

A validated ecosystem for chronic & rare respiratory disease management amongst these: COPD, asthma, bronchiectasis, chronic cough, pulmonay fibrosis, pulmonary hipertension, LONG-COVID, Alpha-1-Antitrypsin deficiency, cystic fibrosis… that combines:

  • Clinical protocols: Digitalized monitoring for pulmonary rehab, symptom tracking, medication adherence, physical activity, respiratory physiotherapy, anxiety, comobidity key indicators, lifestyle.
  • Education: Educational contents about respiratory diseases.
  • Social health: Peer support communities and expert-led education.
  • AI-driven insights: Predictive analytics for exacerbation risk and personalized care plans.
  • Hybrid care model: Blends in-personal assessments (e.g. spirometry via Check@ir events) with virtual coachin when these activities can be carried out.

How it works

The HappyAir Digital Ecosystem can be implemented and used through a step-by-step, hybrid digital-physical integration model, designed for scalability across different healthcare systems and patient populations.

Implementation Steps

Patients Onboarding:

  • Patients are identified via:
    • Clinical referrals from pulmonologists or primary care providers.
    • Screening using Check@ir tool.
  • Onboarding includes consent, language preference setup, and initial health assessment.

Personalized Digital Care Plan Activation: Once enrolled, patients receive a tailored care plan including:

  • Daily symptom tracking.
  • Medication reminders and adherence monitoring.
  • Personalized exercise programs (digital pulmonary rehabilitation).
  • Educational content delivered via app (videos, infographics).

Remote Monitoring & AI-Driven Insights:

  • Patients use the mobile app to log data (symptoms, medication use, physical activity, mood, adverse events, oxygen saturation…).
  • Integration with Bluetooth spirometers and wearables enables automatic physiological data collection.
  • AI algorithms analyze trend to predict exacerbation risk and alert clinicians.

Tele-Coaching and Clinical Intervention:

  • Respiratory physiotherapists conduct scheduled video coaching sessions.
  • High-risk patients flagged by AI are priritized for early intervention.

Community engagement:

  • Join disease-specific groups (e.g. COPD, asthma… by language).
  • Whatsapp Communities.
  • Participate in R&D studies.
  • Test devices or digital health IoT.

Clinician Access & Workflow Integration:

  • Healthcare providers access a real-time dashboard showing patient metricts.
  • Data is viewable within hospital EHRs via HL7/FHIR-compliant APIs (in pilot phase).
  • Alerts help to prioritize follow-ups and reduce preventable.

Hybrid Care Events: Periodic in-person assessments include spirometry and clinical exams, complementing virtual care.
