Short description
The Scirocco Exchange Maturity Model for Integrated care is based on a self-assessment tool design to evaluate the level of maturity in twelve dimensions. It was developed in the context of the project Scirocco to assess the level of integrated care to identify areas of strengths and gaps in the capability to implement integrated care in a country or region. It is useful and intended to involve a wide range of stakeholders spanning from policymakers, health and care managers and professionals, and digital health providers. It is implemented to develop consensus among stakeholders of the areas to prioritise action and advance health and care integration. The online self-assessment tool is open and one can see the results of previous assessments made public.
