United4Health: telehealth services for diabetes, chronic obstructive pulmonary disease, and congestive heart failure

Keywords: Chronic Obstructive Pulmonary Disease (COPD), Diabetes, Heart Failure, Self-Management, Telehealth
Short description
The United4Health project (U4H) tested telehealth services targeting the three main chronic diseases found among the EU population: diabetes, COPD, and CHF. It has the aim to provide an alternative way to support self-management through the provision of an appropriate level of tele-monitoring that is flexible and can respond with fluctuations in their health status and thus avoid future emergency hospital admissions. The telemonitoring package includes video conferencing and a pulse oximeter.
Maturity
There is evidence for economic viability and/or of benefits to the target group of the practice. Further research/development is needed for routine use.
