The twinning will focus on transferring the BONVITA digital platform for effectively screening, preventing and managing frailty and pre-frailty. BONVITA supports the co-design of shared care plans between various stakeholders: geriatric patients, doctors, nurses, physiotherapists, dieticians.
It enables smart planning, lifestyle monitoring, alerts, medication management, education and training, recommendations and reporting. For the adopter, the twinning will offer the opportunity to learn more about and meet the needs of end-users. For the originator, the twinning will enable additional development and customisation of the platform, furthermore, the refinement of its business model and business plan prior to commercialisation.
