The Problem

The Solution
CORēHealth is the regional operational center for telemedicine of chronic diseases and clinical networks in Apulia Region, Italy. Jointly developed by Dedalus and AReSS Puglia, CORēHealth offers: HL7-FHIR interoperable web-cloud platform for care providers; televisit, teleconsultation, telemonitoring, telecooperation and teleconference services; device tracking system; patient synoptic; central alarms; and a mobile App for citizens (available for Android and iOS). Each patient is assigned a disease specific Integrated Care Pathway (“PAI”) and a medical device kit for the detection and monitoring of key vital parameters which are communicated to the doctor in real time. CORēHealth is in exercise since January 2022. Currently, the Center operates in the oncological, haematological, and rare disease networks.
