The twinning will focus on exchanging knowledge around digital tools that support the integration of health and social care services, by facilitating communication between health and care actors, creating new care models and promoting person-centred care and self-management. In particular, the partners will focus their knowledge exchange activities on challenging aspects of digital service design that they have all identified as being critical to the successful implementation and exploitation of digital health technologies. Through the twinning activities, the partner regions have a unique opportunity to drive forward the digital agenda that is required to support health and social care integration and help to ensure that the opportunities that digital can bring are fully realised in adopting regions.
